Re-enrollment for returning Horizons students is now open. See below and check the Remind App for more information. Summer 2026 Dates are 6/24/26 -7/31/26

• Most Recent Medical Form (Most Recent One)
  Formulario médico más reciente (dentro del último año)

o   If the appointment is scheduled before June 24, please notify us.
Si su hijo/a tiene cita antes del 24 de junio, por favor, notifíquenos.

o   Forms must be submitted in person, scanned by email, or uploaded to Salesforce.

o   ❌ Screenshots will no longer be accepted.

o   If your child has allergies or medication, updated authorization and action plans are required.Si su hijo/a tiene alergias o toma medicamentos, se requieren autorizaciones y planes actualizados. Please complete and sign the first page of the physical.
